All forms filled out should be mailed to: City of St. Joseph, 1100 Frederick, Room 107, St. Joseph, MO 64501. They can also be presented in person at the same address. If you have any questions in filling out the forms, please call (816) 271-4751.

Liquor Forms

Application for a Liquor License
This form is used for new businesses who intend to sell liquor. If there is only a change in the corporate filing status, the last two pages are filled out.
Instructions for filling out a Liquor License Application
Caterer's License
This form is used only for temporary catering of liquor at an off site location by a current liquor licensee.
Picnic License
This form is used by Not-for-Profit organizations for temporary events. A 501C-3 or 501C-6 form must accompany the application.

Festival Form

Festival Application Form
This form must be filled out by all organizations seeking to hold a festival, parade, or event in which streets are closed and other assistance is needed by the City of St. Joseph. It should be submitted to the City Festival Coordinator at least 30 days in advance in order to coordinate all information with all City Departments.

Business License Forms

Business Application
This application is filled out by all new businesses prior to issuance of a business license. This form is also used for Health Permit requests. The fee submitted is based upon the business license category you fall under. Please look at the available categories under Listing of Business License Categories and Business License Fee Schedule or call (816) 271-4751 if you have any questions as you fill out this form.

Parking Ticket Forms

Parking Citation Dismissal Request
This form is used to protest any non-moving parking citation. The City Prosecutor will review and return your request with an opinion with 5-7 business days. If your request is denied, you then have an option to request a trial in the St. Joseph Municipal Court.
Parking Ticket Request for Trial
This form is used to request a trial in the St. Joseph Municipal Court. Once the request is received, you will receive a notice from the Municipal Court Division advising you of when your court date has been scheduled.
